> BPC-157 is a synthetic fragment of a human gastric protein that survives gastric juice and shows protective effects across tendon, ligament, gut, and nerve tissue in rodent studies. Human evidence is nearly absent: one retrospective knee series (n=12), one small open-label IV safety pilot, and no published Phase II results. It sits in FDA Category 2, cannot be 503A-compounded, and is banned in competition by WADA.

> **Aliases:** PL 14736, PL-10, PLD-116, Bepecin, PCO-02 **Sequence:** Gly-Glu-Pro-Pro-Pro-Gly-Lys-Pro-Ala-Asp-Asp-Gly-Leu-Gln-Pro-Arg (15 amino acids, MW 1419.5 Da) **Origin:** Synthetic 15-aa fragment derived from human gastric juice "body protection compound" protein **First described:** Sikiric et al. (1992, Croatian team, University of Zagreb)

## Overview

BPC-157 is a stable pentadecapeptide fragment of a naturally occurring gastric protein (BPC) that maintains mucosal integrity. Unlike most therapeutic peptides, BPC-157 is **not destroyed in human gastric juice** , which enables oral administration — a rare property for a 1.4 kDa peptide. It has demonstrated pleiotropic, systemically protective effects across virtually every organ system in extensive (mostly rodent) preclinical work since the early 1990s.

Despite thousands of preclinical citations and explosive off-label/gray-market use, BPC-157 has **no FDA approval** and **no large-scale RCT**. As of September 2026 it sits in FDA **Category 2 (significant safety concern)** — meaning 503A compounding pharmacies cannot use it as a bulk substance. It is banned in-competition by WADA, NFL, UFC, MLB, NHL, NBA, NCAA, NAIA, PGA.

**Evidence Level:** ⚠️ Extensive preclinical animal data; **one** retrospective human clinical series (intraarticular kne, n=12, Lee 2021); **one** open-label IV safety pilot (Lee & Burgess 2025, n small); no published Phase II RCTs as of 2026-09 ([NCT07437547](https://clinicaltrials.gov/study/NCT07437547) registered Feb 2026, no results).

## Mechanism of Action (per Vasireddi 2025 systematic review, PMID 40756949 )

The Vasireddi et al. 2025 HSS J systematic review (n=36 studies, 35 preclinical + 1 clinical) catalogued the following interconnected pathways modulated by BPC-157:

### Pro-survival / Pro-repair Signaling

Pathway | Effect
---|---
**VEGF** | ↑ protein & gene expression — angiogenesis
**ERK1/2 + downstream (c-Fos, c-Jun, Egr-1)** | ↑ phosphorylation — cell growth, migration, angiogenesis
**AKT** | ↑ phosphorylation — pro-survival
**KRAS** | ↑ gene expression — pro-proliferation
**FAK–paxillin** | ↑ in tendon fibroblasts — direct cell migration cue
**Growth hormone receptor** | ↑ expression & activity (tendon fibroblasts in vitro)

### Vascular

  * **NOS / NO** pathway — ↑ eNOS gene/protein, ↑ NO production → vasodilation, angiogenic drive
  * **Collateral pathway recruitment** (azygos vein direct flow delivery in rodent vessel-occlusion models — long-standing Sikiric observation)

### Anti-inflammatory

  * **COX-2** ↓ gene expression
  * **MPO** (myeloperoxidase) ↓ activity
  * **IL-6, TNF-α** ↓ levels

### Neurotransmitter Modulation (Sikiric rodent work)

  * Dopamine, serotonin, GABA, glutamate systems modulated region-specifically
  * ↑ serotonin synthesis in substantia nigra, medial anterior olfactory nucleus
  * ↓ serotonin synthesis in dorsal thalamus, hippocampus, lateral geniculate, hypothalamus
  * Blocks amphetamine-induced startle/stereotypy

## Pharmacokinetics (2025 data)

  * **Plasma half-life: <30 minutes** (rat/dog; He et al. 2022 PMID [36509839](https://pubmed.ncbi.nlm.nih.gov/36509839/); reaffirmed Vasireddi 2025)
  * **Hepatic metabolism** dominant
  * **Oral bioavailability:** BPC-157 retains activity orally in rodents despite gastric stability — but the 2025 MDPI biopharmaceutical assessment (Karami et al., _Pharmaceutics_ 2025, MDPI 18/5/625) explicitly notes this does **not** equate to clinically meaningful oral bioavailability: no intestinal-stability data in simulated intestinal fluid, no Caco-2 / PAMPA permeability assays, no hepatic extraction data have been published. The implicit assumption "gastric-stable → oral works" is unsupported by direct evidence.
  * **Topical / intraarticular routes:** activity demonstrated in animal models; Lee 2021 retrospective used intraarticular kne injection.

They tell you a molecule is worth the cost and complexity of a proper human trial. They do not substitute for one."

## Regulatory Status (2026)

Authority | Stance | Year
---|---|---
**FDA (US)** | **Category 2** bulk substance — significant safety concerns; cannot be 503A-compounded; final 503A recommendation voted 2026-07-23 (see [FDA Peptide Reclassification 2026 — Regulatory Intelligence](fda-peptide-reclassification-2026.html)) | 2023

## Modified & Stabilized Analogues (2025 status)

Commercial "modified BPC" preparations (N-terminal acetylation, C-terminal amidation, retro-inverso, salt forms) circulate in gray-market channels claiming extended half-life or 10× potency. Per Jóźwiak et al. 2025 (_Pharmaceuticals_ 18/2/185, MDPI):

> "So far, there are only a few studies aimed at determining the beneficial properties of the modified analogues of BPC 157."

These modified analogues have **not** been independently characterized for PK, efficacy, or safety. Buyer-beware: molecular weight alone does not predict activity, and chemical modifications alter receptor binding. **Evidence Tier: 5 (commercial/anecdotal only).**

## Uses

Purpose | Notes
---|---
Tendon / ligament / joint | Often stacked with [TB-500 (Thymosin Beta-4)](tb-500-thymosin-beta-4.html)
Gut / GI repair | Oral effect GI-specific in animals; uncertain systemic bioavailability
Muscle recovery |
Systemic healing | —

## Safety Profile

Aspect | Assessment
---|---
Acute toxicity (animal) | Extremely low — no LD50 at 6 μg/kg → 20 mg/kg range (dogs, rats)
Human safety data | Effectively absent at therapeutic doses; case reports of fatigue, GI upset, dizziness (Vasireddi 2025 case-series citation)
Cancer theoretical risk | Angiogenic + GH-receptor activity → theoretical concern. **Not supported** in animal studies, but no long-term human data
Drug interactions | Not characterized
Contraindications | Active malignancy (theoretical), pregnancy/lactation (no data), active bleeding (theoretical)
WADA | Banned in-competition
FDA | Category 2

### Off-label / Gray-Market Use Concerns (Vasireddi 2025)

  * 12–58% prevalence estimates in surveyed athletic populations
  * "Cash clinics" offer intraarticular and oral formulations; quality unregulated
  * No DEA scheduling → legal possession but no quality oversight
  * 50M+ social-media video views (YouTube + TikTok, June 2024); 100k+ Reddit peptide community members
  * SARM parallel: similar gray-market trajectory → likely future regulatory tightening

## Stacking

Stack | Rationale
---|---
**BPC-157 + TB-500** | BPC = cellular signaling; TB-500 = cellular migration. Standard "Wolverine" stack for soft-tissue repair — see [Wolverine Stack - TB-500 + BPC-157 Evidence](tb-500-and-bpc-157-combination.html)
**BPC-157 + GHK-Cu** | Combined skin/wound healing; see [GHK-Cu (Copper Peptide)](ghk-cu-copper-tripeptide-gene-modulatory-regenerative-peptide.html)
**BPC-157 + CJC-1295/Ipamorelin** | Muscle-recovery contexts (no high-quality evidence base)

## Notes / Open Questions

  * **No human RCT efficacy data exists.** All mechanistic and dosing rationale is preclinical transfer.
  * **Hepatic metabolism + <30 min half-life** = exposure window much shorter than the multiple-day tissue repair biology it claims to drive. Mechanism for sustained effect unresolved.
  * **Oral bioavailability quantitatively unmeasured in humans.** Animal "stable in gastric juice" ≠ systemic absorption.
  * **Modified analogues** sold commercially are not independently characterized; treat as distinct compounds.
  * **[NCT07437547](https://clinicaltrials.gov/study/NCT07437547) (Phase II hamstring)** will be the first rigorous human efficacy dataset — watch 2026–2027.
  * **Cancer angiogenesis risk** is the most credible theoretical concern; no animal signal but no human long-term data.
