> Cerebrolysin is a standardized hydrolysate of pig-brain proteins, about 80% free amino acids and 20% small peptides, given IV for stroke, traumatic brain injury, and dementia in dozens of countries. The evidence is a genuine split: two EVER-funded RCTs report benefit in moderate-severe TBI, while Cochrane found weak, biased support in vascular dementia and no benefit plus a safety signal in acute stroke. There are zero controlled trials in healthy people.

> A heterogeneous porcine brain-derived peptide mixture used in stroke, TBI, and dementia across ~40-50 countries (never FDA-approved). library classification: a **natural organ hydrolysate** — pharmacologically and conceptually distinct from Khavinson's short synthetic bioregulators, though often confused with them. Human evidence is a field of contradictions: positive manufacturer-adjacent trials and meta-analyses against two skeptical Cochrane verdicts (weak/biased in dementia; no benefit with a safety signal in stroke).

## What It Is

Cerebrolysin® (EVER Neuro Pharma, Unterach, Austria) is produced by standardized enzymatic hydrolysis of purified young pig brain proteins. The result:

  * **~80% free L-amino acids** (all CNS amino acids present)
  * **~20% low-molecular-weight peptides** (<10 kDa), **> 100 oligopeptides** identified by NanoLC-MS characterization (Yang et al., J Chromatogr B, 2023, PMID [38659966](https://pubmed.ncbi.nlm.nih.gov/38659966/))
  * Contains magnesium, phosphorus, potassium, selenium in trace amounts
  * Lipid-free; fragments small enough to cross the blood-brain barrier — the pharmacological justification whole endogenous neurotrophins (large proteins) cannot claim

This is the same genus of preparation as Russian organotherapy (Cortexin is the Russian porcine cortical peptide analogue), but Cerebrolysin is an Austrian product with the largest clinical trial base of any brain hydrolysate — 200+ trials since the 1970s.

## Proposed Mechanisms

Based on Rejdak, Sienkiewicz-Jarosz, Bienkowski & Alvarez (Medicinal Research Reviews, 2023) and cell-line work:

  1. **Neurotrophic factor mimicry** — peptide fragments mimic activity of **BDNF, NGF, GDNF, CNTF**. The mixture both supplies trophic-like signals and appears to stimulate endogenous neurotrophin production.
  2. **Neuronal differentiation** — robustly induces Neurofilament-L (NF-L) expression in PC12 cells; this is the standard bioassay for its activity.
  3. **Neuroprotection / anti-apoptosis** — reduced neuronal death in models of ischemia, pilocarpine-induced seizure (hippocampal protection, Frontiers Neuroscience 2020), and closed-head injury.
  4. **Synaptic plasticity** — increased dendritic spine density and dendritic length in animal models.
  5. **Angiogenesis, antioxidative and anti-inflammatory effects** — claimed across the mechanistic literature.

**Heterogeneity caveat:** because it is a biological extract, "mechanism" is really a description of a mixture's aggregate behavior. Which peptides are active, in what proportion, and whether batch-to-batch composition matters is under-characterized — a structural weakness shared with all organ hydrolysates.

### Counterfeit / Quality Problem

A comparative bioassay study (Balogh et al., J Clin Neurosci / [PMC11080511](https://www.ncbi.nlm.nih.gov/pmc/articles/PMC11080511/), 2024) tested competing "cerebroprotein hydrolysate" products from other manufacturers (including a deproteinized calf blood extract): **competitor preparations lacked relevant biological activity and had significantly different peptide composition** versus Cerebrolysin. Gray-market "Cerebrolysin" from non-EVER sources — exactly what a biohacker buying peptide websites encounters — has no verified activity. Same lesson as [Peptide Sources and Quality](peptide-sources-and-quality.html).

## Clinical Evidence — Separated by Condition

### Vascular Dementia — Cochrane: Promising but Unreliable

Cochrane review (Chen et al., PMID [31710397](https://pubmed.ncbi.nlm.nih.gov/31710397/); 6 RCTs, n=597):

  * **Result:** courses of IV Cerebrolysin improved cognition and global function vs control, no adverse-effect signal
  * **Verdict:** data _not definitive_ — heterogeneity, high risk of bias, effects "may be too small to be clinically meaningful"
  * No new trials since the prior review; authors call for adequately powered, robust trials

Guekht et al. (J Stroke Cerebrovasc Dis 2011) is the highest-quality positive trial in this space — from a research group with extensive EVER collaboration history (see Incentives below).

### Acute Ischemic Stroke — Cochrane: No Benefit, Safety Signal

This is the hardest evidence in the file. Cochrane review (Ziganshina et al., CD007026):

  * **Moderate-certainty:** no benefit on all-cause mortality; no difference in total serious adverse events
  * **Verdict:** "we would not recommend its routine use in acute ischemic stroke"

Counterpoints (all lower-certainty): \- Strilciuc et al. 2021 safety meta-analysis ([PMC8708612](https://www.ncbi.nlm.nih.gov/pmc/articles/PMC8708612/)) concluded acceptable safety post-stroke — but overlapping author networks with the manufacturer-adjacent trial ecosystem \- 2025 Indian prospective observational study (Medicina, MDPI): improved outcomes as adjunct — uncontrolled design, explicitly acknowledged as such by its own authors \- Thrombectomy subgroup analyses claim reduced hemorrhagic transformation — exploratory only

**Reading:** when the only positive evidence in a condition is observational and manufacturer-adjacent, and the independent systematic review sees harm signal, treat stroke use as unsupported.

### Traumatic Brain Injury — The Most Defensible Indication

  * **CAPTAIN I** (Poon, Muresanu et al., Neurol Sci 2020, PMID ~32095755): randomized, placebo-controlled, double-blind, Asia-Pacific; reported improved outcomes in moderate-severe TBI with early Cerebrolysin
  * **CAPTAIN II** (2023): replicated in a European population
  * Ongoing: [NCT06052787](https://clinicaltrials.gov/study/NCT06052787) — Cerebrolysin + amantadine in ICU TBI patients
  * Caveat: the CAPTAIN program is EVER-funded; TBI has no approved neurorehabilitative drugs, so regulatory bar and publication bias both cut in favorable directions

### Cognitive Enhancement in Healthy Adults — No Evidence

Zero controlled trials in healthy humans. All biohacking/longevity use (e.g., Bryan Johnson's documented self-experimentation) is N-of-1 extrapolation from disease populations — a category error this library rates 1-2 on the Source Evaluation Framework.

## Uses

From trial protocols and non-US product monographs:

**Risk profile:** headache, dizziness, GI symptoms; the stroke Cochrane non-fatal SAE signal (above); rare seizure/encephalopathy reports; porcine-origin hypersensitivity. Not FDA-approved; **not legal for sale in the US** — all US supply is imported gray market, compounding-pharmacy, or counterfeit (see quality section above).

## Evidence Grading Summary

Claim | Level | Source quality
---|---|---
Neurotrophic-like activity in vitro/animals | Strong | Independent cell-line replication (PC12/NF-L)
TBI benefit (moderate-severe, acute) | Moderate | 2 large RCTs, manufacturer-funded, Cochrane-grade review absent
Vascular dementia benefit | Weak-moderate | Cochrane: positive but biased/heterogeneous, possibly sub-clinical effect
Acute stroke benefit | Not supported | Cochrane moderate-certainty: no benefit, non-fatal SAE increase
Healthy-human cognitive enhancement | None | N-of-1 only
Competitor products bioequivalent | Refuted | Comparative bioassay 2024

## Open Questions & Counter-Evidence

  1. If non-fatal SAEs rose 2.4× in stroke (RR 2.39), why did safety meta-analyses conclude "safe"? Partially population/schedule differences, partially author-network overlap — an unresolved tension worth tracking.
  2. No independent (non-EVER-funded) large RCT exists in any indication.
  3. Why did 50 years of use in 40+ countries never produce a Western regulatory approval? Partly commercial (limited patent runway for hydrolysates), partly evidence quality.
  4. The Bryan Johnson-style longevity adoption predates any evidence in healthy aging — monitor for epi-clock or biomarker data, rate as T2 anecdote until then.
