> Dental plaque is the archetypal biofilm, and periodontitis is a chronic biofilm-protected infection affecting 10–15% of adults severely. The strongest systemic link is diabetes: periodontal therapy lowers HbA1c ~0.3–0.4% in meta-analyses of RCTs. Cardiovascular links are associative, the P. gingivalis–Alzheimer's hypothesis failed its direct drug test, and regular antiseptic mouthwash use measurably raises blood pressure by killing nitrate-reducing bacteria.

> Dental plaque is the original biofilm — the subject the entire [biofilm field](biofilms-and-chronic-infection-overview.html) grew from — and periodontitis may be the most underweighted chronic infection in health optimization. The mouth is a permanent biofilm reservoir seeding systemic inflammation, and the oral microbiome is an active endocrine organ (nitrate→nitrite→NO). This file covers the oral-systemic evidence honestly: strong for diabetes links and mechanisms, associative for cardiovascular, unproven-but-mechanistically-plausible for Alzheimer's — including the failed Phase 3 that tested it directly.

## The Oral Biofilm Ecosystem

  * **~700 bacterial species** colonize the human mouth; supragingival plaque, subgingival biofilm, and tongue coating are distinct ecosystems
  * Dental plaque is the archetype of the four-stage biofilm cycle (attachment → EPS → maturation → dispersion) described in [Biofilms Overview](biofilms-and-chronic-infection-overview.html)
  * **Subgingival biofilm** (below the gumline, anaerobic) drives periodontitis: a chronic, polymicrobial, biofilm-protected infection of the tooth-supporting tissues
  * **Keystone pathogen concept:** _Porphyromonas gingivalis_ is present in low abundance but remodels the whole community toward dysbiosis — it subverts host immunity (complement, IL-8) so the commensal biofilm becomes inflammatory

### Periodontitis Prevalence

Severe periodontitis affects ~10-15% of adults globally (WHO); mild-moderate disease affects the majority over 40. It is a stealth infection by this library's definition — chronic, culture-negative in routine care, rarely painful until advanced, and systemically ignore. It belongs on the [Stealth Pathogens](stealth-pathogens.html) watchlist as much as any tick-borne organism.

## The Alzheimer's Link — Evidence and Its Failure

### The Case (Mechanistic)

Dominy et al. (Science Advances, 2019, PMID [30988396](https://pubmed.ncbi.nlm.nih.gov/30988396/)) — the foundational paper:

  * _P. gingivalis_ **DNA and gingipain antigens found in AD post-mortem brains** , significantly more frequent than controls
  * Gingipains (cysteine proteases Kgp, RgpA, RgpB) are the virulence factors; they are neurotoxic, degrade tau-protection pathways, and promote Aβ42 production in vivo
  * Oral gingipain inhibitor (COR388/atuzaginstat) blocked neurodegeneration, reduced brain bacterial load and Aβ42 in mice
  * Proposed routes into brain: hematogenous spread during periodontal bacteremia, infected monocytes, peripheral nerve pathways, and outer-membrane vesicles

Subsequent reviews (Huang et al., Front Immunol 2025; Sarmiento-Ordóñez et al., 2025, [PMC12043021](https://www.ncbi.nlm.nih.gov/pmc/articles/PMC12043021/); MDPI Life 2025) add: gingipain-mediated BBB disruption, LPS-driven neuroinflammation, tau hyperphosphorylation, and epidemiological associations between periodontitis and AD risk.

### The Crash (Clinical)

**GAIN trial** (Cortexyme, [NCT03823404](https://clinicaltrials.gov/study/NCT03823404)) — the direct test of the causal hypothesis:

  * Phase 2/3, randomized double-blind placebo-controlled, **n=643** , mild-moderate AD, 48 weeks
  * Atuzaginstat 40mg or 80mg BID — orally bioavailable, BBB-penetrant gingipain inhibitor
  * **Result: failed both co-primary endpoints** (ADAS-Cog11, ADCS-ADL) in October 2021
  * Safety: hepatotoxicity signal → **FDA clinical hold** (Jan 2022) → development program suspended

**Interpretation for this library:** the human causal test failed. Remaining live options: (a) gingipains contribute to AD initiation decades before symptoms — undetectable in a 48-week symptomatic trial; (b) the association is reverse causation or confounding (poor oral hygiene as early dementia symptom); (c) real but small contribution. Rate the P. gingivalis→AD hypothesis **plausible, unproven, no pharmacological shortcut available**. Prevention (below) remains rational on other grounds regardless.

## Cardiovascular Disease

Mechanistic reviews (Wang et al., Int J Dent 2025, [PMC12398414](https://www.ncbi.nlm.nih.gov/pmc/articles/PMC12398414/); Clinical review Biomedicines 2025):

  * Pathways: periodontal bacteremia → endothelial dysfunction; systemic IL-6/CRP elevation; bacterial DNA/LPS in atherosclerotic plaque (P. gingivalis and Aggregatibacter actinomycetemcomitans DNA detected in plaque samples); autoimmunity to heat-shock proteins
  * **Essa et al. (Br Dent J, 2025):** global federated real-world database analysis — T2D + periodontitis carries **increased CV risk** beyond T2D alone
  * Meta-analyses of cohort data show periodontitis associated with incident ASCVD, stroke (Dewan et al., Dent Med Probl 2024), and dyslipidemia (BMC Oral Health 2024)

**Evidence grade: associative with strong mechanism, causation unproven.** Periodontal-treatment→CV-outcome RCTs do not exist (and may never — required n and duration are prohibitive). Treat as terrain optimization, not established risk-factor treatment.

## Diabetes — The Strongest Causal Evidence

The bidirectional periodontitis-diabetes axis has actual intervention trials:

  * **Meta-analyses of RCTs:** non-surgical periodontal therapy reduces **HbA1c by ~0.3-0.4%** (mean difference −0.34%, 95% CI −0.43 to −0.26) in T2D patients with periodontitis — comparable to adding a second-line oral agent
  * **Baseline HbA1c matters:** systematic review (Adv Ther 2021) — in patients with baseline HbA1c >9%, periodontal therapy reduced HbA1c by **WMD −1.20%** (95% CI −1.68 to −0.72)
  * Oliveira et al. (J Clin Periodontol 2023): 6-month follow-up meta-regression confirms durability of effect
  * Umezaki et al. 2025 ([PMC11893427](https://www.ncbi.nlm.nih.gov/pmc/articles/PMC11893427/)): HbA1c **and CRP** both fall — consistent with infection-burden mechanism, not just oral hygiene theater
  * Diabetes also impairs periodontal defense (AGE-mediated, neutrophil dysfunction) — the relationship is genuinely bidirectional

**Evidence grade: RCT-backed.** This is the strongest oral-systemic link in the file. Any T2D or prediabetic protocol that ignores periodontal status is incomplete.

## The Nitrate-Nitrite-NO Pathway (Why Antiseptic Mouthwash Backfires)

The oral cavity is not just a threat — it is a metabolic organ:

  1. Dietary nitrate (leafy greens, beetroot) is absorbed, concentrated in salivary glands, secreted into the mouth
  2. **Tongue-dwelling nitrate-reducing bacteria** (predominantly _Neisseria_ , _Rothia_ , _Veillonella_ , _Actinomyces_ spp.) convert nitrate → nitrite
  3. Swallowed nitrite → gastric and systemic **nitric oxide** — a major NO source when endothelial NO synthase declines with age

**Antiseptic mouthwash breaks step 2:**

  * Chlorhexidine 0.12-0.2% destroys up to ~90-94% of oral nitrate-reducing bacteria
  * Crossover studies: 7-day CHX → oral nitrite **−90%** , plasma nitrite **−25%** , **systolic/diastolic BP +2-3.5 mmHg** (correlating with nitrite fall)
  * Systematic review/meta-analysis (2025, [PMC13309218](https://www.ncbi.nlm.nih.gov/pmc/articles/PMC13309218/)): twice-daily CHX significantly raises systolic BP after 1 week; nitrate-reducing taxa recover after cessation
  * Older hypertensive patients on PDE5 inhibitors or nitrates have amplified reasons to preserve this pathway
  * Emerging direction (U Exeter group): nitrate as an oral **prebiotic** — beetroot juice shifts oral ecology (↓ _Prevotella_) and supports nitrite production

**Practical inversion:** the biohacker default of daily antiseptic rinse is, on current evidence, mildly harmful for vascular health. Reserve CHX for short therapeutic courses (e.g. post-surgical 1-2 weeks); never chronic.

## Practical Context

Layer | Action | Evidence basis
---|---|---
Mechanical biofilm disruption | Electric brush 2×/day; interdental brushes/floss daily — the only proven daily biofilm control | RCT-grade periodontology consensus
Professional | Periodontal evaluation incl. pocket depth (CDC/AAP case definitions); scaling/root planing where indicated — **especially if T2D** | HbA1c RCT meta-analyses (above)
Antiseptics | **Avoid daily CHX/alcohol rinses** ; short courses only | NO-pathway crossover trials, 2025 SR
Nitrate support | Leafy greens/beetroot intake to feed nitrate-reducing tongue flora | Exeter trials, NO literature
Testing (optional) | Salivary/periodontal pathogen panels exist (qPCR for P. gingivalis etc.) — clinical actionability unvalidated; oral microbiome commercial tests = low T-level evidence | 00 - Meta/Source Evaluation Framework
Adjuncts (frontier) | Intraoral [ozone](ozone-therapy.html) (periodontal pocket insufflation — small trials, RU/EU literature); photobiomodulation for periodontal healing (see 03 - Red Light Therapy/Clinical Evidence); phage endolysins against streptococci — preclinical | Rate 2-3, not standard of care

**On rapamycin/mTOR caution:** any rapa-containing longevity protocol (see [Rapamycin Dosing Protocols](rapamycin-mtor-inhibition-pearl-trial-evidence-and-clinical-translation.html)) with impaired wound healing needs aggressive dental stabilization _before_ starting — dental procedures during rapa courses carry wound-healing and infection risks flagged in the longevity literature.

Periodontitis ↔ systemic inflammation (CRP, IL-6), bacteremia | Strong mechanistic + human
Periodontitis → incident CVD/stroke | Associative, mechanism plausible, no outcome RCT
P. gingivalis/gingipains → AD | Mechanism strong; direct pharmacological test **failed** (GAIN); unproven
Antiseptic mouthwash ↑ BP via NO disruption | Crossover RCTs + 2025 SR; effect ~2-3.5 mmHg
Nitrate as oral prebiotic | Early human trials

## Open Questions

  1. Does aggressive periodontal therapy in mid-life change AD or CVD incidence? (No trial; 20+ year horizon — watch registry-data natural experiments.)
  2. Do gingipain inhibitors with better safety (COR588 successors) ever re-enter testing — perhaps in prevention rather than symptomatic AD?
  3. Which tongue taxa are the load-bearing nitrate reducers, and can they be deliberately seeded (oral probiotics beyond S. salivarius K12)?
  4. Interaction: does rapamycin-induced immunomodulation change periodontal disease trajectory (case reports of gingival overgrowth on mTOR inhibitors in transplant)?
