> The Wolverine Stack combines BPC-157 and TB-500 for injury recovery. BPC-157 has extensive rodent data and five small uncontrolled human reports totaling under 80 subjects, all from one research group; TB-500 has zero published human trials as of September 2026. No study has tested the combination against either peptide alone. Neither is FDA-approved, both are WADA-prohibited, and a July 2026 FDA advisory vote recommending 503A compounding inclusion is non-binding pending rulemaking.

## Overview

The "Wolverine Stack" (BPC-157 + TB-500) is one of the most discussed peptide combinations in the biohacking space for musculoskeletal injury, joint health, and gut healing. Peter Attia gave BPC-157 a 9/10 evidence grade for gut/tendons/inflammation (AMA #83, April 2026) while noting TB-500 evidence is weaker. As of July 2026 the FDA's Pharmacy Compounding Advisory Committee (PCAC) voted to recommend both compounds for inclusion on the 503A bulks list, but the recommendation is non-binding and FDA must complete formal rulemaking (12–18 months historical norm) before either becomes legally compoundable.

## Evidence Separation

### BPC-157 (Body Protection Compound-157)

  * **Animal / In-Vitro** : Extensive rodent data showing accelerated tendon-to-bone healing, ligament repair, gut mucosal protection, reduced inflammation, and angiogenesis via multiple pathways (VEGF, eNOS, etc.). Consistent benefits in models of injury, colitis, and neurodegeneration.
  * **Human Evidence (cumulative 2024–2026)** : Total published human studies = 5 (per FDA PCAC briefing July 2026), **< 80 subjects combined across all five**. No completed RCTs. All lead-authored by Edwin Lee (single research group).

**Human pilot studies (complete record as of Sep 2026):**

  1. **Intra-articular knee pain (2021) — PMID[34324435](https://pubmed.ncbi.nlm.nih.gov/34324435/)** — Lee E, Padgett B. _Altern Ther Health Med_ 2021;27(4):8–13. Retrospective chart review, single-center (Institute for Hormonal Balance, Orlando). 17 patients with chronic knee pain (16 reached for follow-up); 4 mg (2,000 µg/mL) intra-articular injection. 12/12 BPC-157-only patients (91%) reported significant relief at 6–12 months. _Note: a 2025 systematic review (HSS Journal) observed this pilot co-administered thymosin beta-4, confounding attribution to BPC-157 alone._ No control group.
  2. **Interstitial cystitis (2024) — PMID[39325560](https://pubmed.ncbi.nlm.nih.gov/39325560/)** — Lee E, Walker C, Ayadi B. _Altern Ther Health Med_ 2024;30(10):12–17. Intravesical injection in 12 patients with moderate-to-severe IC; 10/12 (83%) reported complete resolution, 2/12 (17%) reported 80% reduction. Single-site, no control. Limited by short follow-up and alternative-medicine publication venue; cystoscopy procedure itself may have contributed.
  3. **IV safety / pharmacokinetics (2025) — PMID[40131143](https://pubmed.ncbi.nlm.nih.gov/40131143/)** — Lee E, Burgess J. _Altern Ther Health Med_ 2025\. Two healthy adults (58-yr M, 68-yr F). Day 1: 10 mg IV over 1 h; Day 2: 20 mg IV over 1 h. No adverse events. No clinically meaningful changes in vitals, ECG, comprehensive metabolic panel, or thyroid function. Plasma returned to baseline within 24 h. Provides only basic safety signal — 2-person pilot, no control.
  4. **Rectal enema — ulcerative colitis (Ruenzi 2005, conference abstract only)** — 53 patients (26 active), 80 mg enema daily × 14 days for mild–moderate UC. Well tolerated; efficacy never published beyond abstract.
  5. **Rectal enema — Phase 1 tolerability (2002–2003, unpublished beyond FDA review)** — Up to 2 mg/kg BPC-157 enema in 24 healthy volunteers; only mild headache and flatulence reported.
  6. **Cancelled Phase I ([NCT02637284](https://clinicaltrials.gov/study/NCT02637284))** — PharmaCotherapia 2015, Tijuana. Designed for n=42 healthy volunteers 18–35. Trial status "Unknown"; reportedly cancelled in 2016 with no results published.

**FDA PCAC 2026 review (BPC-157)** — FDA briefing document (July 23, 2026, 68 pages): "We found no studies that administered BPC-157 to humans via the proposed oral, SC, nasal, or transdermal ROA." Agency recommended against adding BPC-157 to the 503A bulks list; the committee voted 8 yes / 6 no / 1 abstention (both free base and acetate forms) to override FDA's recommendation. Use evaluated: ulcerative colitis only (FDA declined to evaluate tendonitis, Crohn's, celiac — nominations "did not include sufficient information"). The vote does NOT add BPC-157 to the list; FDA must now complete notice-and-comment rulemaking.

**Hudson Biotech Phase 2 hamstring trial ([NCT07437547](https://clinicaltrials.gov/study/NCT07437547))** — first posted Feb 2026; randomized, double-blind, placebo-controlled Phase 2 of pentadecapeptide BPC 157 vs placebo for acute grade II hamstring strain confirmed by MRI. Target n=120; treatment arm daily injection × 14 days plus standardized rehab. Co-primary endpoints: time to return to unrestricted sport; change in MRI-assessed injury volume at Day 14. Sponsor: Hudson Biotech (BH-BPC157-MUSC-001); site: Shenzhen Hospital. Expected completion Feb 2027. Industry-funded.

  * **Key 2025–2026 preclinical additions** :
  * Matek D et al. _Pharmaceutics_ 2025;17(1):119 — **PMID[39861766](https://pubmed.ncbi.nlm.nih.gov/39861766/), PMCID [PMC11768438](https://www.ncbi.nlm.nih.gov/pmc/articles/PMC11768438/)**. Native peptide per-oral therapy reversing quadriceps muscle-to-tendon function loss in a rat model. First dedicated per-oral BPC-157 muscle healing study; supports oral-route plausibility but pre-clinical only.
  * Jozwiak M et al. _Pharmaceuticals_ 2025;18(2):185 — multifunctionality / patent / medical-application literature review.
  * "Regeneration or Risk? A Narrative Review" — [PMC12446177](https://www.ncbi.nlm.nih.gov/pmc/articles/PMC12446177/) (2025). Identifies VEGFR2, Akt-eNOS, ERK1/2 mechanism pathways. Concludes robust clinical trials lacking; human safety "remains unknown."
  * ACG 2025 abstract (S808) — "Oral Peptide BPC-157: An Emerging Adjunct to …" — review of preclinical GI benefits; concludes no clinical safety data available.
  * **HSS Journal 2025 systematic review (DOI[10.1177/15563316251355551](https://doi.org/10.1177/15563316251355551))** — 36 orthopedic studies of BPC-157; 35 preclinical; only human study (Lee 2021) co-administered thymosin beta-4 → cannot attribute effect to BPC-157 alone.
  * Sikiric et al. _Pharmaceuticals_ 2026;19:463 — BPC-157 hemorrhage/thrombosis review (animal data).
  * Mendias. Approved and unapproved peptides. _Sports Med_ 2026.

**BPC-157 free base vs acetate** — distinct active ingredients commonly conflated in marketing; FDA flagged this in 2026 PCAC materials.

### TB-500 (Thymosin Beta-4 fragment)

  * **Animal / In-Vitro** : Promotes actin sequestration, cell migration, wound healing, and reduces inflammation/scar formation. Systemic effects noted in animal models.
  * **Human Evidence** : Zero published human trials of TB-500 fragment as of Sep 2026. All data extrapolated from preclinical or Thymosin Beta-4 parent protein research (RegeneRx RGN-259 ophthalmology program). Distinguish TB-500 fragment from full-length Tβ4 — clinical program is on full-length Tβ4 only.
  * **FDA PCAC 2026 review (TB-500)** — voted 8 yes / 6 no / 1 abstention (free base and acetate forms) on July 23, 2026, to recommend inclusion on the 503A bulks list. FDA's own briefing (media 193349) had recommended against. Non-binding; rulemaking pending.
  * **RegeneRx Tβ4 cardiac program** — 2025 cardiac RCT referenced in third-party literature; full-length Tβ4 only, not TB-500. TB-500 has no comparable clinical program.
  * **[NCT07487363](https://clinicaltrials.gov/study/NCT07487363) (March 2026)** — registered Phase 2 trial of TB-500 17-23 fragment; details sparse.
  * **WADA status** : TB-500 and full-length Tβ4 are both prohibited under Section S0 (non-approved substances) of the 2026 WADA Prohibited List — both in-competition and out-of-competition.
  * **Key Citations** : Reviews in The Conversation (2025), PeptideMind analysis (2026).

### Combination ("Wolverine Stack")

  * **Evidence** : No dedicated animal or human studies testing the specific combination vs. monotherapy. Synergy is hypothesized (BPC-157 local/gut/vascular + TB-500 systemic/actin) but remains speculative.
  * **Clinical Reality** : Popular in peptide clinics for injury recovery; patient reports common but uncontrolled. No evidence the stack outperforms individual use.

## Regulatory Context (2026) — MAJOR UPDATE

  * **July 23–24, 2026 PCAC vote** : BPC-157, KPV, TB-500, MOTS-c recommended for 503A inclusion (all voted 8-6-1 except MOTS-c 7-5-2); Epitalon 7-4, Semax 8-5; Emideltide rejected 6-7. All votes on free base and acetate forms separately. FDA's own scientists recommended AGAINST all seven peptides.
  * **What the vote does NOT do** : It does not change the law. FDA must now complete notice-and-comment rulemaking (12–18 months historical norm). Until then, BPC-157 and TB-500 remain "not on the 503A bulks list" — legally compoundable only if FDA exercises enforcement discretion case-by-case.
  * **State boards** : Have begun auditing clinics using BPC-157 with license suspensions reported in 2024–2025 (state actions still in force pending federal rulemaking).
  * **HSS Journal 2025** identified 36 orthopedic BPC-157 studies, 35 preclinical. The single human study co-administered thymosin beta-4 → confounding attribution.
  * **Route mismatch** : FDA explicitly stated no human studies via the routes people actually buy (oral, SC, nasal, transdermal). All 5 published human studies used rectal enema, intra-articular, intravesical, or IV routes.
  * **BPC-157 specifically** : FDA Category 2 bulk substance (Oct 2023). Finalized Feb 2024 — compounding pharmacies can no longer legally produce BPC-157 for human use without 503A inclusion. PCAC July 2026 vote is the path back but rulemaking remains.
  * No approved indications for either in humans in any country. Use remains investigational/off-label.
  * WADA: TB-500 and Tβ4 prohibited S0 (all times). BPC-157 prohibited S0 (all times). Athletes subject to anti-doping rules cannot use either regardless of prescription status.
  * Emphasis in sources on distinguishing marketing claims from evidence.

## Claims Audit

Clinic operators and users should avoid unsubstantiated claims of "rapid healing," "Wolverine-level recovery," or anti-aging effects. Evidence supports only preliminary animal data for tissue repair; human data insufficient for efficacy or safety conclusions in stacks. Long-term risks (including theoretical oncogenicity or unknown interactions) unstudied.

**Human-evidence-claim sanity check (Sep 2026):** Any marketing line implying "clinically proven," "FDA-approved," or "FDA-cleared" for BPC-157 alone or in stack is **unsupported**. The strongest defensible language is "investigational; animal data encouraging; preliminary pilot human data suggests short-term tolerability at limited doses; PCAC July 2026 recommended inclusion on 503A bulks list but FDA rulemaking pending." Claims about specific routes (oral, SC, nasal, transdermal) are unsupported by any human study per FDA's own July 2026 review.
