> Vitamin D deficiency affects about 42% of US adults and far more in dark-skinned populations, and this entry argues the 30 ng/mL threshold marks adequacy against rickets rather than optimal health. The evidence splits by baseline status: meta-analyses find lower cancer and all-cause mortality mainly in deficient or older subgroups, while VITAL and D-Health found no benefit in largely replete populations. Mendelian randomization supports a causal role for low vitamin D in MS, type 1 diabetes and mortality.

> **Core thesis:** Vitamin D deficiency is near-universal in modern populations, yet optimal levels (likely 40-60+ ng/mL, not the inadequate 30 ng/mL threshold) may be critical for immune function, hormonal health, and disease prevention. The mainstream 600-800 IU recommendation is grossly insufficient for most people.

## The Deficiency Crisis

### Why Everyone Is Deficient

  * **Sun avoidance:** Modern indoor lifestyles, sunscreen use, latitude north of 37° eliminates most vitamin D synthesis
  * **Dark skin:** Melanin blocks UVB conversion; dark-skinned individuals need 5-10x more sun exposure
  * **Aging:** Skin's 7-dehydrocholesterol decreases with age; seniors synthesize 75% less vitamin D from same sun exposure
  * **Obesity:** Vitamin D is fat-soluble and sequestered in adipose tissue, reducing bioavailability
  * **Gut issues:** Malabsorption syndromes, celiac disease, Crohn's impair absorption

### Prevalence

  * Estimated 1 billion people worldwide have vitamin D deficiency
  * 41.6% of US adults are deficient (<20 ng/mL)
  * 82.1% of Black Americans and 69.2% of Hispanics are deficient

## Optimal Blood Levels

Serum 25(OH)D | Classification | Implications
---|---|---
<10 ng/mL | Severe deficiency | Rickets (children), osteomalacia (adults)
10-20 ng/mL | Deficiency | Increased fracture risk, immune dysfunction
20-30 ng/mL | Insufficiency | Suboptimal for many physiological functions
**30-50 ng/mL** | Conventional normal | Inadequate for optimal health per many researchers
**50-80 ng/mL** | **Optimal range** | Associated with lowest all-cause mortality
80-100 ng/mL | High-normal | May be optimal for some conditions
>100 ng/mL | Potential excess | Monitor for toxicity (rare)

### The 30 ng/mL Myth

The 30 ng/mL threshold represents the level needed to prevent rickets/osteomalacia, not optimal health. Many functional medicine practitioners argue: \- Observational studies show U-shaped curve with lowest mortality at 50-70 ng/mL \- Autoimmune diseases show dose-response relationship up to higher levels \- GrassrootsHealth data suggests 60-80 ng/mL for optimal immune function

## Doses in studies

### Loading Phase (8-12 weeks)

For severe deficiency (<20 ng/mL):

### Maintenance Phase

After reaching target levels:

Target Level | Daily Maintenance
---|---
40 ng/mL | —
50 ng/mL | —
60+ ng/mL | —

### Doses in studies

  * **Daily > weekly > monthly:** Daily dosing produces more stable blood levels
  * **Studies administered with fat:** Vitamin D is fat-soluble; absorption increases 50%+ with fatty meal
  * **Higher doses are safe:** Up to 20,000-40,000 IU/day shown safe for months in studies; toxicity only at 100,000+ IU/day sustained

## Form Comparisons

### D2 (Ergocalciferol) vs D3 (Cholecalciferol)

Aspect | D2 | D3
---|---|---
Source | Plant/ fungus (ergosterol) | Animal (lanolesterol)
Potency | ~50% of D3 | 100% potency
Half-life | Shorter (~24 hours) | Longer (~48 hours)
Evidence | Weaker clinical data | Stronger RCT data
Recommendation | Avoid | Preferred

### Delivery Formats

Form | Absorption | Notes
---|---|---
**Oil-based capsules** | Best | Olive oil, MCT oil bases
**Liquid drops** | Very good | Precise dosing, sublingual possible
**Softgels** | Good | Common, affordable
**Tablets** | Variable | May have absorption issues
**Sprays** | Emerging | Sublingual absorption, less data

## Critical Co-Factors

### Vitamin K2 (Essential)

  * Vitamin D increases calcium absorption
  * Without K2, calcium deposits in arteries instead of bones
  * **Recommendation:** 100-200 mcg K2 (MK-7 form) per 5000 IU vitamin D
  * Synergistic effect on bone density and cardiovascular health

### Magnesium (Essential)

  * Magnesium is required for vitamin D metabolism (conversion in liver/kidneys)
  * Deficiency limits vitamin D effectiveness
  * Supplementing vitamin D without magnesium can deplete magnesium stores
  * **Recommendation:** 300-500 mg elemental magnesium (glycinate, citrate, or malate)

### Vitamin A

  * Works synergistically with vitamin D

* * *

## Genetic Considerations

Gene | Polymorphism | Impact
---|---|---
**CYP2R1** | Various SNPs | Impaired 25-hydroxylation (reduces vitamin D activation)
**GC** (vitamin D binding protein) | Variants affect free vitamin D levels | May need higher total vitamin D
**CYP27B1** | 1-alpha hydroxylase variants | Impaired conversion to active form
**VDR** | Receptor polymorphisms | Variable response to supplementation

* * *

## Uses

  1. **Baseline test:** 25-hydroxyvitamin D (25(OH)D), not 1,25(OH)2D
  2. **Re-test:** 8-12 weeks after starting/changing dose
  3. **Maintenance:** Annual testing once stable
  4. **Optimal timing:** No fasting required; can test any time of day
  5. **Lab ranges:** Request actual number, not "normal/not normal"

## Clinical Evidence

Condition | Evidence | Findings
---|---|---
**Bone health** | Strong | Reduced fractures at >30 ng/mL; meta-analysis of Ca+D supplementation confirms BMD benefit in postmenopausal osteoporosis (BMC Musculoskelet Disord 2025;26:928)
**Autoimmune** | Moderate-Strong | —
**Respiratory infections** | Moderate | Reduced URI incidence; COVID-19 benefit confirmed in 2025 umbrella review (Petakh et al., Front Nutr 2025;12:1559471)
**Long COVID** | Emerging | RCT (Atieh 2025, MDPI Nutrients 17:304) showed K2+D3 reduced fungal translocation & inflammation markers in long COVID
**Cardiovascular** | Mixed | Lower CVD risk in observational studies; VITAL/VITAL-ancillary neutral for major CVD events
**Cancer incidence** | Mixed | No consistent reduction in incident cancer across VITAL, D-Health, ViDA
**Cancer mortality** | Moderate-Strong | **2025 DKFZ meta-analysis (14 RCTs, >100k participants): daily D3 400-2000 IU reduced all-cause cancer mortality 12% (RR 0.88, 95% CI 0.79-0.98); up to 16% reduction in age >70 and baseline <20 ng/mL**
**Sepsis/critical illness** | Emerging | Multiple 2024 RCTs of calcitriol in sepsis (Sistanizad 2024, Thampi 2024); 2026 meta-analysis (Front Nutr) shows mortality benefit in deficient ICU patients
**Mendelian randomization** | Causal | Genetically-predicted 25(OH)D inversely associated with MS, T1D, all-cause mortality — supports causation beyond confounding
**Mortality (observational)** | Strong | U-shaped curve; lowest mortality at 50-70 ng/mL (Pilz 2019, reanalysis 2025)
**Depression** | Moderate | Correlation with mood; some RCT improvement

### Critical Reading of Negative Trials

VITAL (25,871 participants, 2,000 IU/d, 5.3y) and D-Health (21,315, 60,000 IU/month) showed **no all-cause mortality benefit in largely vitamin-D-replete populations** (baseline mean ~30 ng/mL). This does NOT contradict benefit in deficient populations — it shows population-level sufficiency matters more than universal supplementation. Trials enrolling only deficient patients consistently show benefit.

## Contraindications & Cautions

Condition | Guidance
---|---
**Hypercalcemia** | Do NOT supplement
**Kidney disease** | Monitor closely; may need active form (calcitriol)
**Sarcoidosis** | Monitor; may over-convert to active form
**Granulomatous diseases** | Monitor calcium
**Certain medications** | Thiazides, corticosteroids, anticonvulsants — consult provider

## Special Populations

### Pregnancy

  * Deficiency associated with preeclampsia, gestational diabetes, low birth weight
  * Target: maintain >40 ng/mL throughout pregnancy

### Elderly (>70)

  * 2025 DKFZ meta-analysis: 16% cancer mortality reduction in this subgroup
  * Higher baseline deficiency → larger absolute benefit
  * Pair with weight-bearing exercise and K2 for fracture prevention

### Dark-skinned populations

  * 82% of Black Americans deficient (<20 ng/mL)
  * Same physiological endpoints require 5-10x more sun exposure OR 2-3x oral dose
  * Mendelian randomization studies confirm low 25(OH)D causal for adverse outcomes in this group

### Critically ill / ICU

  * 60-80% of ICU patients deficient
  * High-dose enteral or IV cholecalciferol improves outcomes in deficient subgroup (2026 meta-analysis)
  * Calcitriol (active form) trials: Sistanizad 2024, Thampi 2024 show reduced infectious biomarkers
