Klotho

The most credible resilience factor in geroscience, and still zero human dosing data.

Updated 14 Oct 202614 sources10 min read
Short answer

Klotho is a protein made mainly in kidney and brain that falls with age; low levels track higher mortality and cognitive decline in large cohorts. In animals the evidence is strong: overexpression extends mouse lifespan about 20-30%, and a single low-dose injection improved memory in aged monkeys. But no human has received klotho in a published trial, and the gene-therapy outfits selling it, including a non-placebo plasmid pilot, are running ahead of the science.

Summary drafted from this entry’s sources; editorial review pending.

01Key questions

Has klotho been tested in humans?

Not as a treatment. All human data is observational or genetic. A non-placebo plasmid pilot (NCT07216781) is running without published results, and an FDA orphan-designated AAV program for ALS is still pre-IND.11

Does klotho extend lifespan?

In mice, yes. Genetic overexpression extends lifespan about 20-30%, and a single AAV dose encoding secreted klotho extended wild-type mouse lifespan 15-20%. There is no human lifespan data.612

Does klotho improve cognition?

A single low-dose injection improved memory in 18 aged rhesus monkeys, and higher human klotho levels associate with better cognition and attenuated Alzheimer's biomarkers in APOE4 carriers. The high dose failed in monkeys, so more is not better.4568

What raises klotho levels?

Aerobic exercise is the best-supported lever in meta-analyses of trials; vitamin D repletion, smoking cessation and phosphate moderation also associate with higher klotho. No approved drug raises it.8

Is klotho gene therapy safe?

Unknown, and the signals argue caution: high-dose klotho failed in monkeys and homozygous carriers of the KL-VS variant fare worse, implying a narrow therapeutic window. The marketed plasmid uses permanent overexpression, the opposite of the transient pulse that worked in primates.611

Klotho is a kidney- and brain-derived protein whose overexpression extends mouse lifespan ~20–30% and whose circulating (soluble) levels decline with age in humans, tracking mortality, kidney decline, and cognitive resilience. A single low-dose injection improved memory in aged monkeys (2023), making klotho one of the most credible "resilience factors" in geroscience — but zero human interventional data exist as of Aug 2026. What exists instead is a fast-moving, largely unregulated gene-therapy pipeline (Minicircle plasmid trial NCT07216781; Klotho Neurosciences KLTO-202) selling ahead of the science.

03Discovery and Basic Biology

Klotho was discovered in 1997 when Kuro-o and colleagues serendipitously generated a mouse with a disrupted gene producing a striking premature-aging phenotype: growth retardation, arteriosclerosis, osteoporosis, skin atrophy, infertility, and early death (Kuro-o et al., Nature 389:45–50, 1997). The gene was named for Klotho, the Greek Fate who spins the thread of life.

Two functional forms exist:

  1. Membrane klotho — a co-receptor for FGF23 (phosphate/vitamin-D homeostasis), expressed mainly in kidney tubules and choroid plexus. Its loss explains the phosphate-retention phenotype of klotho-deficient mice.
  2. Soluble klotho (sKlotho) — the extracellular domain shed by ADAM10/17 proteases, circulating endocrinely. This is the form measured in serum and the form implicated in cognition and resilience.

A key mechanistic puzzle: peripherally administered klotho does not appreciably cross the blood-brain barrier, yet enhances hippocampal function — implying an indirect peripheral signaling relay (platelet factors and immune modulation are candidate mediators) rather than direct CNS action.

Mechanisms (segregated by evidence type)

Mechanism System Evidence
FGF23 co-receptor; phosphate wasting, vitamin-D catabolism Membrane klotho, kidney Extensive mouse genetics

04Human Evidence

Genetics — the heterozygote advantage

The KL-VS variant (F352V) of the KL gene: heterozygous carriers (~20–25% of populations) have higher circulating klotho and show better cognition and larger brain volumes in some cohorts; homozygosity is deleterious — a classic overexpression toxicity signal. Heterozygote advantage is the field's standing caution against "more klotho is always better" logic.

Serum levels (observational)

  • Circulating sKlotho declines with age in large cohorts (NHANES).
  • Mortality: low serum klotho associated with all-cause mortality in population-based adults (Kresovich et al., 2021; PMCID PMC9122743, cited >95×). A 2024 analysis derived operating cutoffs: klotho <548.8 pg/mL → ~52% higher all-cause mortality risk; <660.9 pg/mL → ~58% higher (Liu et al., Sci Rep 2024; DOI 10.1038/s41598-024-52701-4).
  • Kidney disease: lower serum klotho linked to all-cause mortality, cardiovascular mortality, and adverse renal outcomes in CKD meta-review (Kanbay et al., 2024; PMCID PMC11398896); inverse association with diabetic-kidney-disease prevalence and mortality (Xie et al., Diabetol Metab Syndr 2025; DOI 10.1186/s13098-025-01729-1).
  • Cognition: klotho levels associate with executive function/dementia risk cross-sectionally; causality unproven.

⚠️ Assay caveat: serum sKlotho ELISAs are not standardized across manufacturers; absolute cutoff values are not portable between labs. sKlotho is a research biomarker, not a clinical-grade diagnostic.

Animal intervention evidence

  • Lifespan: klotho overexpression extends median mouse lifespan ~20–30% (Kurosu et al., Science 309:1829–33, 2005).
  • Cognition (mice): klotho elevation (genetic or protein) enhances learning/memory across aging, Alzheimer's, and Parkinson's models via NMDAR/synaptic mechanisms; klotho overexpression rescues cognitive decline in human-amyloid mice (Dubal et al., Cell Rep 2014; DOI 10.1016/j.celrep.2014.03.076, PMID 24746767).
  • Cognition (aged primates): a single low-dose (10 µg/kg) systemic klotho injection improved memory-domain cognition in 18 aged rhesus macaques within ~4 hours, lasting ~2 weeks (Dubal et al., Nature Aging 3, 2023; PMID 37400721). Critically, the high dose (100 µg/kg) did not work — the dose-response is non-monotonic. This is the single most important preclinical result driving clinical interest.

052025–2026 Evidence Update

The past 18 months materially expanded the klotho literature. Headline additions:

Uses

The most consequential 2025 publication. Roig-Soriano et al. (Mol Ther 33(4):1449–65, 2025; PMID 39988871; PMCID PMC11997498) treated wild-type C57BL/6 mice at 6 or 12 months with a single intravenous AAV9 vector encoding the secreted klotho splice form (s-KL). Outcomes at 24 months (~70 human years):

  • Lifespan: median increase 15–20% (Kuro-o's earlier 30–40% result used full-length genetic overexpression; this is the first AAV-based, single-dose, secreted-only replication).
  • Muscle: larger fibers, less fibrosis, improved regeneration.
  • Bone: preserved trabecular structure (osteoporosis-protective signal), more pronounced in females.
  • Brain: increased hippocampal neurogenesis and immune activity.

Independent commentary notes KL injections at younger age produced lower serum KL than at 12 months — implying the youthful klotho-axis is already at capacity, and only the aging host benefits from supplementation. This re-frames "klotho as anti-aging drug" as a deficiency-replacement, not supra-physiological boost, model — directly undercutting the permanent-overexpression plasmid logic of Minicircle.

Cardiometabolic comorbidity (NHANES 2025)

Wang et al., Sci Rep 15:16722 (2025; DOI 10.1038/s41598-025-01580-4), n=11,198 NHANES 2007–2016 participants 40–79 yrs:

  • α-Klotho inversely associated with CVD, obesity, CKD, NAFLD.
  • U-shape for T2DM, L-shape for CKD.
  • OR for cumulative metabolic comorbidity burden in CVD = 0.56 (95% CI 0.35–0.91).
  • CKD mediates ~20% of the klotho → CVD association.
  • Higher klotho → lower all-cause mortality only among those with CMDs (interaction), not healthy participants — consistent with klotho as a stress-response / resilience factor, not a generalized longevity tonic.

Cognition / dementia — three converging 2025–2026 papers

  1. Yang et al. 2026 (Alzheimers Dement 22(4):e71397, PMID 41996134, PMCID PMC13089206), n=354 older adults with Aβ-PET, tau-PET, plasma p-tau217, GFAP, NfL, cognition: plasma klotho attenuates AD biomarkers and cognitive decline — but only in APOE ε4 carriers. Plasma klotho × APOE-ε4 interaction significant for p-tau217, GFAP, NfL, Aβ-PET, cognition. AD biomarkers mediate the cognitive protection. This is the first concrete human evidence of a klotho × APOE-ε4 synergy in AD.
  2. Katonova et al. 2025 (Alz Res Ther; DOI 10.1186/s13195-025-01878-5) — KL-VS heterozygosity associated with α-klotho protein levels and AD risk modification in aMCI/AD dementia.
  3. Raber et al. 2025 (Front Aging Neurosci 17:1599402; DOI 10.3389/fnagi.2025.1599402) — CSF α-klotho higher in controls than dementia patients; correlated with Aβ40/Aβ42 in a species- and cognitive-health-dependent fashion; estrogen HT increased CSF α-klotho in ovariectomized rhesus.
  4. Systematic review / meta-analysis (2025) — α-Klotho, serum and plasma levels all associated with cognition / dementia; proposed as a biomarker panel component (Sci Direct S1876201825000607).

CSF klotho is a research, not clinical, biomarker — same caveat as serum

Assay heterogeneity (ELISA manufacturer differences; pre-analytical CSF handling) and lack of a reference standard mean reported absolute values are not portable across labs. Treat klotho as a research-grade aging biomarker for now.

Context: Minicircle's NCT07216781 record cites the Dubal mouse and monkey papers as its scientific basis — but plasmid-mediated continuous overexpression is pharmacologically the opposite of the single low-dose transient pulse that worked in monkeys. Nothing in the primate data supports the dosing logic of a permanent-expression plasmid; combined with homozygous KL-VS toxicity and high-dose failure in monkeys, the risk ledger is unfavorable until independent data appear.

07Raising Klotho Without Gene Therapy (Evidence-Based)

Lever Effect on sKlotho Evidence
Aerobic exercise ↑ moderate; ≥12 weeks chronic aerobic training significantly elevates circulating sKlotho Meta-analyses of RCTs (e.g., de Luca Corrêa et al. 2022, PMC9585050; confirmed in PD cohort 2025, npj Parkinsons Dis); best-supported non-pharmacologic intervention
Vitamin D repletion ↑ (klotho expression is vitamin-D responsive) Human observational + trials
Estrogen / hormone therapy ↑ CSF α-klotho in ovariectomized NHPs (Raber 2025) NHP — translation to human uncertain
Phosphate restriction ↑ / protective Mechanistic + CKD data; avoid excess processed-food phosphate, colas

08Hype Ledger (What Klotho Is NOT)

  • Not a demonstrated human cognitive enhancer — no human has received klotho protein under controlled conditions with published results.
  • Not dose-linearly beneficial — monkey high-dose failure + homozygous KL-VS harm argue for tight therapeutic windows.
  • Not validated as a clinical biomarker — assay non-standardization; association ≠ causation for mortality links.
  • Not an argument for unregulated plasmid clinics — permanent overexpression in healthy 23+ adults has no primate safety basis; Peptide Sources and Quality logic applies with more force to gene therapy.
